How to participate in the Suboxone Program

 If you would like to participate in the Suboxone Program at Aroostook Wellness you will need to take the following steps to be considered:

 

1.) - For insurance purposes, our office MUST receive a referral from your Primary Care Physician. If Dr. Conner is not presently your Primary Care Physician, he is not able to be considered as your Primary Care Physician. You will need to seek out a Primary Care Physician outside of Aroostook Wellness.

2.) - You need to be currently involved in a substance abuse program/counseling.

3.) -  Your substance abuse counselor must be one that works with Aroostook Wellness. Contact our office to find out which counselors work with Aroostook Wellness.

4.) - Your substance abuse counselor must be the person that contacts Aroostook Wellness - NOT the patient.

5.) - Once our office has received a referral from your Primary Care Physician and has been  contacted by your counselor, you will receive notice upon acceptance into the suboxone program.

**Please note: There is limited enrollment into this program, and often a waiting list. Upon acceptance into the program you will sign a detailed contract, agreeing to full accountability. Once the contract is read and signed, if at any time any part of the contract is broken by the patient, THEY WILL BE DISCHARGED from the program with no possibility of re-enrollment. For this reason, and also because the availability of this program is very limited in Aroostook County, it is asked with sincere concern, that a patient be certain they are ready to take on the responsibilities of participating in the suboxone program.
